EyeMed Cares Disaster Relief Replacement Program

A note for EyeMed members:
If a state of emergency is declared in response to a natural disaster1, impacted members can enroll in our short-term EyeMed Cares Disaster Relief Replacement Program to receive vision care and materials in the event urgent assistance is needed.
To enroll:
- EyeMed members simply call our dedicated EyeMed Cares support team at the phone number provided by their employer
- An agent will be standing by2 to collect their first name, last name, date of birth, and email address
- They will receive an email at the address provided that includes a summary of available coverage, eligibility information and instructions for use
Individuals affected by a natural disaster who are not EyeMed members can reach out to the OneSight EssilorLuxottica Foundation at support@onesight.org if assistance obtaining eyewear is needed.
A note for EyeMed network providers:
If a state of emergency is declared in response to a natural disaster1 in your area and you need to update your practice information on our Provider Locator, simply follow the instructions below.
To update your office hours:
- Visit our online claims system
- Click the “Manage My Profile” tab
- Navigate to “Hours of Operations”
- Make your changes and save
If your practice is closed due to a natural disaster1 and you are unable to serve members, you can notify us by completing the Change/Move Location Address Form. Please indicate in the notes how your practice has been affected.

- EyeMed reserves the right to offer the EyeMed Cares Disaster Relief Replacement Program at its discretion based on impact of disasters on a specific community. The Program is only available in identified localities.
